    Facts & Figures about Diabetes

    Diabetes is reaching epidemic proportions. Worldwide, the number of cases is expected to double to 300 million in less than 25 years. Today, over 5 million people worldwide live with Type 1 diabetes (the more severe form of the disease), of which almost 395,000 are children.
    If left untreated, long-term complications of the disease can lead to heart failure, blindness, impotence, kidney failure, amputation, and in many cases death. Tight glycaemic control is central to good health, as proven by numerous studies, including the landmark
    Diabetes Control and Complications Trial (DCCT), which concluded in 1993.

    • Diabetes has become one of the major causes of premature illness and death in most countries, mainly through the increased risk of cardiovascular disease (CVD). Cardiovascular disease is responsible for between 50% and 80% of deaths in people with diabetes.

    What is Diabetes?

    Diabetes prevents the body from properly using energy from food. The condition occurs when the pancreas produces little or no insulin, a hormone required to sustain life, or when the pancreas makes insulin, but the body resists it.

    • Insulin, a hormone required to sustain life
      Insulin is a hormone produced by the Pancreas to aid accessing to the energy stored in the food. Largely this results from the ability of the body, under the influence of Insulin, to release sugar from the food intake, to store it and to use it, when needed, to drive our normal functions.
    • The normal stability of blood sugar levels
      The normal metabolic process of releasing glucose from the food intake ensures that the amount of sugar in our blood remains within the region of 3.5 to 4.5 mmol/l(60-80 mg/100ml). Insulin levels rise when we have a meal in order to capture the sugar from the food intake. Insulin levels reduce after the processing of the food intake over a period of approximately 2 hours. During the rest of the day, an associated mechanism prevents blood sugar levels from falling below the normal levels by releasing glucose from the body storage areas in fat and tissues.
    • The Diabetes disease
      Diabetes is a description of the condition where there is a mismatch between the food intake and the levels of production or utilisation of Insulin.
      • In Type 1 diabetes, the pancreas no longer produces insulin, an important hormone that helps metabolise glucose and supply energy to cells. A person with this condition must administer insulin using injections or an insulin pump.
      • Type 2 diabetes is a metabolic disorder resulting from the body's inability to make enough, or properly use, insulin. Type 2 diabetes is often controlled by diet and exercise, oral medications and, in some cases, insulin. Research indicates that obesity and weight gain are associated with an increased risk of Type 2 diabetes. Insulin pumps have been successfully used to treat Type 2 patients requiring insulin and with poor glycaemic control.

    Without an adequate availability of insulin, a person is unable to get energy from food or to keep glucose levels in balance. Poor glucose control can lead to acute complications from hypoglycaemia (low blood glucose) or hyperglycaemia (a condition associated with higher-than-normal blood glucose), which can lead to ketoacidosis, causing coma or death, if left untreated. Diabetes is the leading cause of blindness, kidney failure and amputation, and is a major factor in cardiovascular disease and nerve damage.

    Therapy Options

    Depending on the type and stage of the disease, different treatment options currently exist:

    For people with Type 2 diabetes who do not require insulin administration, the treatment usually mainly consist in:

    • Diet and exercise
    • Oral medications

    For people with Type 1 diabetes, and those with Type 2 diabetes who require Insulin administration, the treatment consists in insulin injection and requires monitoring the blood sugar level. The difficulty lies in making the best match of insulin dosage and food uptake, along with the normal energy requirements of people's day-to-day living.

    Monitoring blood sugar levels
    In practice, matching Insulin dosage with food uptake requires to see what blood sugar levels are like as well as to identify each patient's patterns in blood sugar swings.

    • Finger pricks
      To achieve a sensible balance most Diabetics monitor their blood sugar levels with one of the many systems available on the market. At the moment all such devices require the patient to draw a small sample of blood, usually by means of a finger prick, place it on a reagent or sensor pad, which is then analysed by a machine. People intensively monitoring their blood sugar typically prick their fingers four or more times per day.
    • Continuous Glucose Monitoring System
      Intermittent measurements taken by finger prick blood tests do not always provide sufficient information regarding when the glucose level is changing, and if so, how fast, and in which direction. The CGMS system enables healthcare professionals to view comprehensive data and glycaemic patterns, which can be useful in managing a patient's diabetes by identifying patterns in blood glucose swings. The CGMS system is intended for occasional rather than everyday use, and is to be used in conjunction with standard blood glucose monitoring.

      The CGMST system contains two key components: an external glucose sensor and a monitor. A healthcare professional inserts the sensor into subcutaneous tissue, typically in the abdominal area. The sensor is typically worn for a period of 24 to 72 hours, and provides electronic signals to the monitor every ten seconds. The monitor averages glucose measurements, and also stores glucose data and event information that has been entered manually by a patient. Healthcare professionals are able to download comprehensive information from the monitor into a computer, enabling them to view retrospective data and proactively adjust diabetes therapy in the patients they treat.     A treatment regime
    A treatment regime will normally incorporate target levels for blood sugars at different times of the day, with a balance being achieved by better matching the amount of insulin in accordance with a suitable diet. The aim of any treatment regime, however, is to maintain blood sugar levels as close as possible to normal levels.

    Insulin administration

    Unregulated high levels of blood sugar are clearly implicated in several conditions. The complications of poorly controlled Diabetes are the single largest cause of End Stage Kidney Failure and loss of vision.
    The most comprehensive study carried out in this area, the Diabetes Control and Complications Trial (DCCT), which concluded in 1993, came to the clear conclusion that: "Over a life time, DCCT-defined intensive therapy reduces complications, improves quality of life, and can be expected to increase length of life".

    • Intensive Conventional Therapy (ICT)
      The evidence is that Intensive Conventional Therapy (ICT) can achieve an appropriate control of blood sugar levels in most individuals who take the trouble to adhere to the requirements. For most people this has meant an increase in the number of injections, often up to four or five per day, along with much more diligent monitoring of blood sugars.
    • Continuous Subcutaneous Insulin Infusion (CSII)
      Insulin pumps, which provide treatment referred to as Continuous Subcutaneous Insulin Infusion are an excellent tool for helping people improve glycaemic control. It is especially indicated in cases where ICT has not achieved good results.
      Insulin pumps have been shown to improve glucose control for many patients and can provide a better quality of life than injection therapies. An insulin pump is able to more closely mimic the action of a pancreas by continuously delivering small doses of insulin, even while a patient sleeps. This is extremely important, as poor glucose control can lead to long-term complications of diabetes, such as blindness, kidney failure, nerve damage, amputation and heart disease.

    Over 70 abstracts and articles on the usage and experience with the CGMS system have been published since the product's introduction in 1999. In fact, successful results from a pilot study were published in the December 1999 issue of Diabetes Research and Clinical Practice, and indicated that patients' HbA1c levels were reduced an average of more than 1% after only five weeks following use of the CGMS system (Bode BW, Gross TM, Thornton KR, Mastrototaro JJ. Continuous glucose monitoring used to adjust diabetes therapy improves glycosylated hemoglobin: a pilot study. Diabetes Research and Clinical Practice 1999;46:183-190). The lead investigator of the pilot study noted that HbA1c levels had never before been lowered so quickly and that therapy adjustments resulting in this reduction would not have been made on the basis of intermittent information obtained through traditional glucose meters. Furthermore, a 10 week follow up showed that HbA1c levels remained as low or lower, without any additional intervention by the healthcare team, thus demonstrating the sustainability of these results.

    Insulin Pump Therapy

    What is an insulin pump?
    An insulin pump is a small, battery-operated device (about the size of a pager) that can replace insulin injections for patients managing diabetes. The purpose of a pump is to provide an accurate, continuous controlled delivery of insulin, in pulses which can be regulated by the user to meet their Glucose control objectives.

    How does an insulin pump work?
    The pump delivers insulin from a reservoir inside the pump, through a thin plastic tube (called an infusion set), to the body. Most infusion sets are worn in the abdominal area and use a tiny, flexible tube, called a cannula, which is inserted easily into the skin using an insertion device. Patients generally refill their insulin and change their infusion sets every two to three days.

    A pump is easy to program and automatically delivers a constant rate of insulin - called a "basal rate"- to keep blood glucose in the desired range between meals and over night. Guided by a physician, the user creates a customized basal pattern, with the goal of closely matching individual needs. Pump users can easily adjust the pump for changes in lifestyle needs, and can program insulin delivery at mealtimes.

    • Pump therapy is simple. It is based on what your body does naturally - deliver small pulses of insulin all the time and little extras controlled by the patient at mealtimes to cover the food intake.
    • The pump helps achieve "tight glucose control." Diabetes patients can easily adjust insulin to keep glucose levels within a near-normal range. A pump can help patients avoid hyperglycaemia (high blood sugar), which can degrade health over time, and hypoglycaemia (low blood sugar), an acute condition that can be very dangerous, particularly at night while a patient sleeps.
    • The pump more closely mimics a healthy pancreas. An insulin pump more closely mimics a healthy pancreas by delivering insulin effectively, continuously and with minimal variability. A healthy pancreas delivers insulin every 10 to 14 minutes. A pump more closely mimics the pancreas by continually delivering insulin around the clock and allowing patients to program multiple basal rates depending on their individual needs.
    • The pump is precise and accurate. Insulin pumps can deliver insulin precisely and accurately in 0.1 unit increments, which is nearly impossible for people using traditional injection therapy.
    • With the pump, people can be more flexible. Unlike patients using longer-acting insulin with injection therapy who must follow rigid schedules of insulin injections, meals and snacks, patients using an insulin pump can program insulin delivery when they eat, and adjust or stop insulin delivery for exercise or other needs. Patients using pump therapy can eat what they want, when they want -- something almost unheard of in patients treating their condition with traditional insulin injections.

    Why good control is important?

    Better blood glucose control has been the focus of a lot of attention since the release of the results of the landmark Diabetes Control and Complications Trials (DCCT) in 1993. The DCCT was the largest long-term research study on the effects of glucose control ever conducted. There were two groups of subjects in the study. One group managed their diabetes using "conventional" methods (1 - 2 injections per day and 1 - 2 daily blood glucose tests), while the other half used "intensive" methods, with frequent blood glucose monitoring and daily adjustments of food and insulin to keep blood glucose as near normal as possible. Nearly half in the intensively treated group used insulin pumps to help them achieve that level of control. The chart shows the differences in control that the different styles of treatment produced.

    The DCCT showed clearly that those who achieved a better blood sugar control with an average 2% lower glycohemoglobin value had a tremendous decrease in their risk for the long-term complications of diabetes. Risk of diabetic eye disease was decreased by 76% and reductions in risk for kidney and nerve damage were nearly as great. Obviously, diabetes control matters... a LOT. In fact, these results were so significant that the DCCT investigators felt compelled to end the study one year early so the conventionally treated patients could have the opportunity to realize the benefits of intensive diabetes management.

    An insulin pump is a good way to achieve the degree of control now known to be the best defense against long-term health problems.

    If the answer to the above question is yes, you would be a frequent user of the weighing scale. Not satisfied with the reading on the weighing scale you might have gone on to calculate the BMI and then gone on to rate yourself as under weight, normal, obese or very obese and tuned your fitness programs accordingly. So far so good..But do you know that while BMI is one of the measure to evaluate your fitness, its not completely reliable.

    What is BMI?

    BMI is a formula that estimates a person’s body fat using only his/her weight and height. The result is then used to determine weight categories: 18.5 and below is considered underweight; 18.6-24.9 healthy; 25-29 overweight and 30+ obese.

    BMI is NOT the right measure....

    Bone mass and muscle mass varies from person to person. So two people aged 30 years and weighing 85 Kgs might have completely different fitness levels. The reason is that the bone mass, muscle mass (both known as lean mass)and fat mass varies from person to person depending on his physical activity level and food habits.

    Also the scale was created years ago and is based on Caucasian men and women. It doesn’t take into account differences in body composition between genders, race/ethnicity groups, and across the lifespan.

    It has been proven that BMI, the number used to indicate weight category does not reflect the same amount of body fat for some races compared to others.

    Therefore its very crital to evaluate the % of body fat present in the indivudual before one can classify him as underweight, normal, obese or very obese. How do you go about doing that? There two very accurate tests that can help calculate your body fat % in the body accurately.

    1. Hydrostatic Testing
      Hydrostatic Testing (Underwater Weighing) has become universally regarded by industry experts as the "Gold Standard" in body fat testing. It works on the fact that bone, muscle, and connective tissue collectively known as lean mass sinks, while body fat floats is the main principle behind (hydro-densitometry) hydrostatic testing. By obtaining your land weight and water weight (based on buoyancy), your body fat as a percentage of your total weight can be scientifically calculated.
    2. DXA Method In this method, dual-energy x-ray absorptiometry, which is a low dose x-ray known as DXA, is used to determine percent fat. DXA can be used to estimate bone density, lean mass and fat mass. The DXA method uses two different frequency X-Rays and rates of absorption of the body and determines the value based on the difference between the two.

    What is the Bioelectrical Impedance method?

    Muscles, blood vessels and bones are body tissues having a high water content that conduct electricity easily. Body fat is tissue that has little electric conductivity. Omron Body fat monitor sends an extremely weak electrical current of 50 kHz and less than 500 µA through your body to determine the amount of fat tissue. This weak electrical current is not felt while operating the BF400. The Bioelectrical Impedance Method safely combines the electric resistance with the distance of the electricity conducted. Correct posture and consistent measuring conditions need to be maintained for the best results.

    DXA (Dual Energy X-Ray Absorptiometry), has been the established method for accurate evaluation of body composition. OMRON has used research information from several hundred people using the DXA method to develop the formula by which the body fat monitors work. The body fat mass and body fat percentage is calculated by a formula that includes five factors: electric resistance, height, weight, age and gender.
